An Application on Execution of The Rights of Data Subjects

If the Administrator is not able to identify the person submitting the application on the basis of the sole application, they will ask an applicant for additional information.
The application can be submitted personally or through the proxy (e.g. a member of the family). Due to personal data safety, the Administrator strongly advices to use an authorization certified by a notary or an authorized legal counsel. This will significantly quicken the process of verifying the authenticity of the application. The application should have: name and surname of the applicant, PESEL number (if applicable), date and place of birth, place of residence (country, place, street, number of the house/apartment), the topic which the application refers to as well as a signature of the applicant.
The response should be sent to the applicant within one month from receiving the application. If case of any setback, the Administrator is obliged to inform the applicant on the reason of such a delay.
The response is sent only via traditional postal service unless the application was submitted via email and an electronic response was previously required.
Any proceedings in terms of submitted applications are free of charge. Charges can be levied only in case of: • a request to issue second or any other consecutive copies of data (first copy is free of charge); in such case the Administrator has the right to charge the applicant with a fee which covers the costs of preparation, service and shipment of requested information. The abovementioned fee includes administrative costs related to the realization of the request. • frequent and excessive requests submitted by the same person or requests which are ungrounded; in such case the Administrator has the right to charge the applicant with a fee which covers the costs of preparation, service and shipment of requested information. The abovementioned fee includes costs of providing communication as well as costs of taking required actions.
If the applicant questions the decision of imposing financial charges, they are entitled to lodge a complaint to the President of the Personal Data Protection Office.
